#GivingTuesday712 Spotlight: Community Health in Council Bluffs

With Giving Tuesday quickly approaching on December 1, SHARE Omaha is spotlighting nonprofit organizations within our community. Proud to feature our #GivingTuesday712 efforts in Council Bluffs, with this blog we highlight the Visiting Nurses Association, All Care Health Center and FAMILY, Inc who are providing essential healthcare services and programs in our community.

VISITING NURSE ASSOCIATION (VNA)

At the Visiting Nurse Association (VNA), the Home Health Care program services are physician-directed and provide the full spectrum of care their patients need, including overall case management, coordination of care, and discharge planning, while focused on continuity of care and outcome achievement. VNA patients continue to present with more complex health needs and complicated socioeconomic challenges, as well as ongoing patient assessment and teaching/education. Skilled nurses, social workers, registered dietitians, and physical, occupational and speech therapists deliver intensive home visitation services to at-risk families living in poverty. This program allows clients the opportunity to receive care from the comfort of their own home.

ALL CARE HEALTH CENTER

All Care Health Center is determined to meet the growing need of healthcare for members in the community who live in poverty and without insurance. Healthcare needs go beyond medical visits and include prescription medicines and durable medical equipment, such as continuous glucose monitors that enable our providers to both monitor and educate our diabetic patients toward a healthier lifestyle. With the help of the community, All Care is seeking to meet the needs of pharmacy medicines and products for uninsured and underinsured patients. In 2019 All Care served 6,129 patients including 50% under the Federal Poverty Level.

FAMILY, INC.

With the help of donations for #GivingTuesday712, FAMILY, Inc will launch a mobile wellness unit. The unit will prove a one-stop-shop for preventative screenings for children and families in the area. The vision of the organization is to create a community of early identification and intervention into developmental concerns for children in Southwest Iowa.
Your financial support during #GivingTuesday712 will assist in receiving and meeting the healthcare needs of our community.
