Community members,

We invite you to participate in the 2024 Community Health Needs Assessment that metro region hospitals, health departments, and other non-profit agencies have asked Professional Research Consultants (PRC) to conduct in our community now through May 31, 2024.  

This assessment is a survey about things people in our community do which may affect their health. It helps us better understand the health needs and challenges in our region so we can work together to address them.  

By participating in this assessment, you contribute to shaping the future of health initiatives in the Omaha metro area.   

There are TWO ways to participate:

Online Survey:  

• The survey is now available at www.prcsurvey.com/begin/MetroOmaha2024 
• IMPORTANT: Each respondent must take the survey by entering with the exact URL noted above.  You cannot copy and share the link from your web browser once a “survey session” has started and a unique session ID is assigned — this will result in an “expired” notice. 
• The online survey will take 25 minutes to complete.  
• Survey is available in both English and Spanish using the same URL/link.  

Phone survey:  

• PRC will be conducting phone surveys and are calling people randomly.
• The survey will take approximately 20 minutes to complete.   
• Phone survey is available in English and Spanish.  

You only need to take the survey once; if you have completed it via phone or online, do not take it again. This helps ensure we have accurate results.
